- Each has two (2) witnesses who can attest to the fact that they are both single.
- If the child is under the age of 18, parental approval is required (if parents are in the Philippines) authenticated by Regional Trial Court, Malacanang Office, and Department of Foreign Affairs, notarized by nearest notary public.
- The National Statistic Office (NSO), Malacanang Office, and the Department of Foreign Affairs, Manila, will authenticate the birth certificate.
- Applicants’ passport photocopies
- Two (2) 2X2 photographs;
